Overview

 Responsible for the assessment of needs and coordination of treatment services for members identified as needing a high level of service coordination, as identified through the use standardized screening tools.   Acts as the primary point of contact for all Child and Family Team (CFT) participants to facilitate meetings and follow-up recommendations, to ensure continuity of care and service delivery, and to educate team members about the CFT process.

 

Responsibilities

 

  • Performs on-going assessment of needs and service intensity for members, using identified tools.
  • Develops a comprehensive CFT to include natural and community supports and professional involvement for children.
  • Facilitates CFT meetings to discuss and identify services needed to meet the treatment needs of members.
  • Develops and monitors service plans developed in collaboration with the CFT.
  • Attends and participates in team meetings, clinical staffings (as needed), court hearings, and other meetings relevant to meeting the treatment needs of members.
  • Provides referrals to outside providers and community resources in accordance with HIPAA policies.
  • Assists with discharge planning for members leaving out-of-home placements, including hospitals and residential facilities.
  • Provides education, support services and rehabilitative skills to members and their families.
  • Maintains a case load size of 20 to 25 complex needs members.
  • Engages members at the required level of service sufficiency for position.
  • Maintains member records in accordance with agency and funding source requirements.
  • Completes progress notes using the appropriate covered service billing codes.
  • Participates in individual and team supervisions as required by licensing, agency, funding source or as requested by supervisor.
  • Completes all training required by licensing, agency, and funding source or as requested by supervisor.
  • Must be able to provide own reliable transportation to and from relevant meetings in the community.
  • Ability to accurately read, record and interpret information, including assessments, measures, and diagnostic criteria.
  • Position may require some evenings and weekends.

 

Qualifications

Education/Experience:

  • Bachelor’s degree in related field preferred, or
  • Bachelor’s degree with at least one year prior experience with at-risk populations.

Legal Qualifications:

  • Arizona Driver’s License, 39-month Motor Vehicle Report, and vehicle registration and liability coverage that meets insurance requirements.
  • 21 years of age.
  • CPR and First Aid certification required (ICHD provides training)
  • Eligible for DPS Level 1 fingerprint clearance required
  • Current negative TB test result required (ICHD provides)

Prefered Qualifications:

  • Prefered bilingual (candidates may be eligible for a language stipend).
