Responsible for the assessment of needs and coordination of treatment services for members identified as needing a moderate level of service coordination, as identified through the use of the CASII. Acts as the primary point of contact for all Child and Family Team (CFT) members to facilitate meetings and follow-up recommendations, to ensure continuity of care and service delivery, and to educate team members about the CFT process.


  • Performs on-going assessment of needs and service intensity for members, using identified tools.
  • Develops a comprehensive CFT to include natural and community supports and professional involvement for children.
  • Facilitates CFT meetings to discuss and identify services needed to meet the treatment needs of members.
  • Develops and monitors service plans developed in collaboration with the CFT.
  • Attends and participates in team meetings, clinical staffings (as needed), court hearings, and other meetings relevant to meeting the treatment needs of members.
  • Provides education, support services and rehabilitative skills to members and their families.
  • Assists with discharge planning for members leaving out-of-home placements, including hospitals and residential facilities.



  • Bachelor’s degree in related field preferred, or Bachelor’s degree with at least one year prior experience with at-risk populations.
  • Arizona Driver’s License, 39-month Motor Vehicle Report and proof of vehicle registration and liability coverage that meets insurance requirements.
  • 21 years of age
  • CPR, First Aid , Article 9 and  Prevention and Support certification required (ICHD provides training)
  • DPS Level 1 fingerprint clearance required(ICHD provides)
  • Physical examination report  including a current negative TB test result (ICHD provides)
  • Must be able to provide own reliable transportation for business purposes.
