Overview

Responsible for the assessment of needs and coordination of treatment services for members identified as needing service coordination. Acts as the primary point of contact for all Child and Family Team (CFT)/Adult Recovery Team (ART) members; to facilitate meetings and follow up recommendations; to ensure continuity of care and service delivery and to educate team members about the CFT/ART process.

Responsibilities

  • Performs on-going assessment of needs and service intensity for members, using identified tools.
  • Develops a comprehensive CFT/ART to include natural and community supports and professional involvement for members.
  • Facilitates CFT/ART meetings to discuss and identify services needed to meet the treatment needs of members.
  • Develops and monitors service plans developed in collaboration with the CFT/ART.
  • Attends and participates in team meetings, clinical staffings, court hearings and other meetings relevant to meeting the treatment needs of members.
  • Provides referrals to outside providers and community resources in accordance with HIPAA policies.
  • Assists with discharge planning for members leaving out-of-home placements, including hospitals and residential facilities.
  • Provides education, support services and rehabilitative skills to members and their families.
  • Completes progress notes using the appropriate covered service billing codes and writes comprehensive and timely reports as required by funding source and/or system partners.
  • Adheres to the agency’s policy on billable hours/encounter expectations.
  • Maintains member records in accordance with agency and funding source requirements.
  • Participates in individual and team supervisions as required by licensing, agency, funding source, or as requested by supervisor.
  • Maintains an approved schedule and acceptable level of attendance.

Qualifications

Behavioral Health Technician (BHT:)

  • Associate’s degree and two years related experience,* or,
  • High School Diploma/GED and three years related experience* required.

              Behavioral Health Technician  II (BHT II):

  • Bachelor’s degree related to behavioral health and related experience* preferred, or
  • Any bachelor’s degree with at least one year of verifiable work experience* required.
  • Certification for counseling, assessing and service planning

            *One year of internships and volunteer work, in a behavioral health or related setting will

              be accepted for related experience
