  • Overview

    The LPN provides nursing care, medical treatment, and helps facilitate the provision of outpatient behavioral health services for members at Intermountain Health Center (IHC).  The LPN performs administrative tasks such as updating client records, obtaining medical histories and interfacing with the Lab.


    • Receives and processes requests for medication refills utilizing nursing log. (Locate chart, request information from other sites if applicable, determine if medication information is correct, complete medication refill progress note after approval by Behavioral Health Medical Practitioners (BHMP) and notify pharmacy.
    • Aids with the maintenance of medical records on all clients, including generating and following up on Incident Reports regarding injury, medication adverse events or other incidents pertaining to medication.
    • Administers injectable medication administration to members as prescribed by BHMP.
    • Obtains Vital Signs for appointments, at Physician’s discretion.
    • Performs chart audits to keep up to date on status of psychiatric evaluations, labs, AIMS testing, or other tests ordered. Pass information to the Medical Assistant/Medical Office Assistant to make BHMP aware of need.
    • Accurately documents when a new medication is started, when patient education is completed. Notes any side effects or adverse reaction and any change in medications.
    • Properly checks in and inventories all samples; disposes of expired samples and records such activities.
    • Meets with patients and families per antidepressant protocol.
    • Maintains an approved schedule and acceptable level of attendance.


    • Education –
    • HS/GED required.
    • Completion of Practical Nurse coursework at an accredited institution.
    • Experience –
    • At least two years of full time behavioral health work experience (childrens’ network experience preferred).
    • Prior experience with Electronic Health Record (HER) preferred.
    • License – Requires current Arizona license as a Practical Nurse (LPN or RN)
    • Regulatory
    • 21 years of age.
    • Current, valid Arizona Driver’s License and 39 month Motor Vehicle Report to meet insurance requirements.
    • Eligible for DPS Level I fingerprint clearance.
    • First Aide, CPR certification (Employer provides).
    • Initial current negative TB test result, within the prior 12 months (Employer provides)
